Private Health Care Services (PHC)

Five Acres partners with private health care organizations across Southern California to provide access to intensive services for children with the goal of maintaining them safely in their own home and community. Through our care, we help lower the child’s risk of psychiatric hospitalization and prevent the need for out-of-home congregate care placements.

• From day one, the child and family are connected with their own team of strengths-based and family-centered mental health professionals. Their PHC team engages the child and family in developing an individualized plan using natural supports and community resources to help the child and family thrive.
• Medication management is an important aspect of mental health treatment, and appointment attendance can sometimes be an issue. That’s why the entire Five Acres PHC team works with the child and family to achieve a 99% attendance rate for psychiatric appointments.
• We believe children heal and grow best within families. Five Acres’ PHC outcomes for permanency—keeping a child connected to a permanent, loving family—are the highest in our agency.
